Seismic pressure gauge trip event sharing
On May 13, 2019, during the normal drilling operation of the XXX drilling team, when it was drilled to a depth of 1906 meters at about 18:00, the remaining 2.6 meters, the pump pressure was 19-21 MPa, and the riser pressure gauge popped out from the pressure gauge seat. . The driller Fan XX heard the sound, observed the puncture drilling fluid, thought it was a leaking pipe, stopped the pump and raised the square drill pipe about 10 meters to start the rotary table drilling tool, and issued a whistle signal. The cadre on duty Liu XX ran out of the post after hearing the sound and ran to the drill floor. After inspection, it was found that the riser pressure gauge popped out from the pressure gauge seat and reported to the captain Wang XX; Wang XX went to the rig to watch the scene Later, the incident was reported to the Drilling Project Department at 18:39. The incident did not cause personal injury or damage to other equipment.
The direct reason is that the riser pressure is higher than the riser pressure-bearing capacity of the riser pressure gauge, and pops out from the pressure gauge seat.
Indirect reason: After the original flanged pressure gauge was damaged, the drilling team ignored the warning of “No Triggering Here” on the original gauge, and installed ordinary threaded pressure gauges on the original flange with inconsistent buckle types.
Management reasons: staff training is not in place, different understanding of flange pressure gauges and ordinary threaded pressure gauges is not clear, the installation is not carefully checked whether the threaded fasteners match.
Precautions
1. When replacing the flange type pressure gauge, the gauge and the flange should be replaced as a whole; it is strictly forbidden to replace the gauge head alone, and carefully check whether the seat and the buckle on the gauge are intact and there are no defects.
2. Replace the pressure gauge to confirm whether the pressure gauge is matched with the gauge screw.
3. Each well team, liquid distribution station and related units immediately checked the installation of similar pressure gauges in their units. Those who have problems should immediately replace them and report in the QHSE Group Daily Report before May 21. The site supervisor urged the well team to investigate.
